Connecticut Home Care Program for Elders

This program assists persons 65 years and older to live in the community and get needed assistance with bathing, dressing, toileting, eating, transferring, medication management and meal preparation. Elders can choose either to self direct their care or have home health agency services. Home delivered meals, companion services, personal financial management services and emergency response system are available. Program includes two levels: Medicaid funded level and State funded level. Those with higher incomes may be required to pay a monthly fee for the services.

Independence Northwest Article Separator Personal Care Assistance Waiver

This Medicaid program assists persons between the ages of 18 and 64 who need assistance with bathing, dressing, eating, toileting, and transferring. Program also provides for an emergency response system.

Independence Northwest Article Separator Acquired Brain Injury Waiver

This Medicaid program assists persons with a traumatic brain injury under the age of 65 who need in home assistance with either hands on care or cueing for bathing, dressing, toileting, eating, and transferring. Program provides personal care assistance, companion services, independent living skills training, and prevocational services as determined by the needs of the consumer.

This Medicaid program assists persons with a history of serious, persistent mental health disabilities to live in the community and receive the necessary services to achieve stability and recovery in their lives. Waiver can provide agency based recovery assistants, home health aides, medication management and mobile crisis services.

Independence Northwest Article Separator Department of Developmental Services Waivers

The Department of Development Services offers two waivers for persons with mental retardation or Prader Willi Syndrome to receive home and community based services in the community setting of their choice. Consumer must be eligible for the services through the Connecticut Department of Developmental Services. Program provides for a wide array of choices in services.

Independence Northwest Article Separator Katie Beckett Waiver

This Medicaid program offers home and community based services generally to children under the age of 18. Program provides for a nurse case manager and agency based home health services.

Independence Northwest Article Separator Connecticut Home Care Program for People with Disabilities

This program provides in home services for persons living with degenerative neurological diseases between the ages of 18 and 64 who are not eligible for Medicaid and who need assistance with at least two critical needs. Income and asset limits do apply. For those over income and asset limits, a monthly fee may be charged to the consumer receiving services provided by the program.

Independence Northwest Article Separator Veterans’ Aid & Attendance

This program provides for a supplemental pension for veterans or their surviving spouses who served during certain wartime eras and who need financial assistance to pay for in home services. Program is administered by the federal US Department of Veterans Affairs.
